Vehicle Information: 2026 Honda CR-V Sport HybridComponent: Power Train / Hydraulic Brakes / Electronic Stability ControlDescription of Incident:On [Date], my 2026 Honda CR-V Sport Hybrid was parked facing uphill on a steep incline. I started the vehicle, depressed the brake pedal, released the electronic parking brake, and shifted into Drive. Upon transitioning to the accelerator, the vehicle failed to hold its position. It immediately and rapidly rolled backward down the incline, completely overriding the Hill Start Assist (HSA) mechanism. Despite accelerator input, the hybrid powertrain failed to deliver the instantaneous forward torque required to counter gravity. The vehicle gained rearward momentum and collided with a closed gate.Safety Justification:This un-commanded rollback presents a severe, systemic safety hazard. …

Additionally, the contact stated that the rearview camera image was black due to the failure. The contact stated that while driving approximately 60-65 MPH, the contact heard a male voice coming from the infotainment system. On 3/17/26, my husband and I started heading N on RT 83 to a medical appointment in our 2025 Honda CR-V Touring Sport. We were doing between 40 to 50 miles/hr. Twenty minutes later, we smelled faint smoke which was attributed to the trucks driving around us. Upon exit to our next route we immediately came to a red light and stopped and found heavy dark smoke coming from inside the front of our hood. We slowly moved to the R lane to get to a strip parking lot to move vehicle to a safe spot. When we finally found a safe parking a vehicle stopped to alert us of a noticeable fire at the bottom front of the Honda. We jumped out and 911 was called. By the time Police, followed by the Fire Dept, the front of my CR-V was engulfed by fire. I had just purchased this 2025 Touring Sport from Carmax 6 months earlier and it had just 7091 miles to it. …
While pulling into a parking spot and coming to a stop, the brakes failed and the car abruptly accelerated forward despite attempts to continue braking. The car proceeded over the curb and raised hilly grassy median and only came to a stop when it collided with a parked vehicle on the other side of the median. I, as the driver, had no control of the vehicle and could have potentially hit a pedestrian in this parking lot. There was a person seated in the car that my car impacted and she could have been hurt as well as myself and my passenger. All of this was reported to the community service officer who came to the scene, as well as to the Coggin collision center, Coggin Honda dealership and my insurance company. My insurance company has sent an appraiser to the collision center but he did not investigate the causative factors, only the body damage so far. …

The contact also received bruises to their neck and knees that required medical attention. A police report was filed. The vehicle was able to be driven the short distance home. The contact then stated that they later had the vehicle towed to the dealer where it was diagnosed that the battery had failed, however, they did not provide a diagnosis for the brake failure. The vehicle had not been repaired. The manufacturer was made aware of the failure. The failure mileage was 2,024.
The contact owns a 2024 Honda CR-V. The contact stated that the first year the vehicle was purchased, while driving at an undisclosed speed, the contact experienced phantom braking. The vehicle was braking independently while there were no vehicles or pedestrians nearby. No warning light was illuminated. The contact stated that on a separate occasion, while driving at an undisclosed speed with the cruise control activated, the vehicle accelerated independently. Woke up at [redacted] to neighbor banging on door and yelling your car is on fire. Vehicle is parked in front driveway, front part of the car where engine is ablazed. Neighbor said he heard "pops" like firecrackers/gunshot when he went to look out his window the fire was reflected on the fence across the car. I did google search for possible fire causes and search returned with Recall notice for fuel pump, battery and battery cladding on model. Contacted Honda but Aileen just promised to send inspector for joint inspection with USAA inspector but hasn't been responsive to call or emails for follow up. Fortuntaely, didn't park in the garage or we would have been burned down with the house also quite fortunate that car was driving on the road. …
