Part Failure Motorcraft FD4615 (secondary filter on top of the engine) It is available for inspection. This occurred approximately 200 miles after installation. Failure resulted in diesel fuel saturating the entire engine compartment, firewall, and underside of the truck. Extreme fire hazard. Failure should be confirmed by mechanic today (it's pretty obvious). No other inspection performed as of yet. No warning lamps/messages/symptoms. Only sign was extreme smell of diesel in the cab. When vehicle stopped, and was inspected, there was diesel dripping from the engine compartment and underside of truck past the transmission. Was driving truck down the road when I turned off on a side road. When I did, there was an extreme smell of diesel. I stopped, and got out of the truck. …
On August 21,2021 we were about 2 hrs from home meeting some family to visit for awhile my husbandand I are both in our 60's. Just as we were entering Lufin Tx our truck dropped from 60 mph to 30 with no warning. I pulled to shoulder of the road and truck completely died. If we had been on an interstate there's a possibility the truck would have died in the middle of the interstate hwy with no way of getting out of traffic. Our truck was towed to Lufkin Ford since we couldn't afford the $ 394.00 to have it towed closer to our home. We were told by Lufkin Ford our P4 high pressure pump had went out, sending metal shards through our motor a $9400.00 repair. To this date our truck has not been repaired because there's a problem getting the pump. …

The contact's son owns a 2013 Ford F-250. The contact stated that the vehicle was parked while working and after returning to the vehicle and attempting to restart the vehicle, the message "Reduced Power" was displayed, and the vehicle was not able to be started. The contact's son had the vehicle towed to an independent mechanic who diagnosed the vehicle and determined that the high-pressure fuel pump had failed, and the fuel system and fuel pump needed to be replaced. The vehicle was repaired. The manufacturer was informed of the failure. The contact researched online and related the failure to NHTSA Campaign Number: 24V957000 (Fuel System, Diesel); however, the VIN was not included. The failure mileage was 206,181.

The contact owns a 2021 Ford F-250. The contact stated that while her son was driving 25 MPH on a slight incline, the vehicle made an abnormal sound and then stalled. Both the contact's son and husband were unable to restart the vehicle after several attempts. There was no warning light illuminated. The vehicle was towed to an independent mechanic, where it was diagnosed that the high-pressure fuel injection pump had failed due to metal shavings intrusion into the fuel system. The vehicle was not repaired. The dealer was notified of the failure and referred the contact to the manufacturer. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 200,000. The vehicle was repaired at the owner's expense.
Hold down clamps for the fuel injectors are breaking or coming loose on diesel engine. Injectors are not visible for a person to look at as they are buried by covers. Due to the injector coming loose it would allow combustion gasses to escape the cylinder causing other injectors to become damaged and possibly causing a fire. This issue has happened 2 times on my truck and both times the Ford Technicians stated that it is not something that should happen. Vehicle was inspected/fixed by Ford dealership Techs back in March of 2021 and again in May of 2021(still there at this time). A check engine light came on and i immediately drove to the dealership both times. I will provide a picture of a Ford tech site where they talk about this issue happening and their internal memo. Ford at this time refuses to fix the issue and is making me pay for their defective parts.
Truck started leaking fuel and then the engine shut down the next day it was drove, while driving! I've received 2 quotes and need the whole fuel system replaced due to CP4 pump failing which will cost about $13,000! This is the same issue as the 24S78 Ford recall but my truck doesn't fall in the correct year! I still owe 10k on this vehicle so it's basically totaled now.
