NHTSA defect investigations
Federal safety investigators have looked into 2 possible defects involving the 2014 Toyota Corolla:
Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited.
Electrical overstress
The Office of Defects Investigation (ODI) opened this investigation to determine if the failure of airbags to deploy during severe crashes, in certain vehicles, was the result of a safety related defect. During the investigation a complex failure was studied that can result in non-deployment of subject vehicle air bags and other restraint system devices in severe crash events.

The contact owns a 2014 Toyota Corolla. The contact stated while driving at 5-10MPH, he blacked out and the vehicle crashed into a tree on the front passenger's side. The contact's roommate transported him to his residence. The contact later sought medical assistance and was informed he had sustained a broken nose and broken ribs. The vehicle was towed to a body shop where it was totaled. A police report was filed. The dealer was not notified of the failure. The manufacturer was notified of the failure and sent an EDR report to the contact. The contact became aware that the VIN was included in NHTSA Campaign Number: 20V024000 (AIR BAGS), which he associated with the failure. …

On March 12, 2025, I was involved in a frontal collision when another vehicle unexpectedly pulled out in front of me. Impact occurred to the front-left area of my vehicle. Despite the nature of the collision, the front airbags did not deploy. The vehicle is subject to Safety Recall 20TA03, which addresses incomplete or non-deployment of airbags and/or seat belt pretensioners due to ECU susceptibility to electrical noise in certain crash scenarios. The vehicle was declared a total loss by my insurance company and is no longer available for inspection. The manufacturer was notified in May 2025 and again in November 2025. The manufacturer closed the case citing inability to inspect the vehicle and later relied on photographs only to rule out airbag deployment concerns. No warning lights or messages related to the airbag system were observed prior to the collision. …

The factory white paint on my 2014 Toyota Corolla has been peeling and delaminating in large sheets down to the primer and bare metal. This appears to be a paint adhesion failure, not normal wear or surface chipping. The vehicle has not been in any accidents, repainted, or subjected to improper care. The problem developed gradually and has continued to worsen over time. Large sections of paint detach from body panels such as the hood, roof, trunk, and sides. Paint has come off while driving and during routine car washing. This condition presents a safety concern because pieces of paint can separate at driving speeds and become road debris, which could strike other vehicles or create hazards for nearby drivers. In addition, exposed metal body panels are vulnerable to corrosion, which may affect the long term safety and integrity of the vehicle. …
