What owners report
I was rear ended while at complete stop by someone going over 40mph on 12/19/22. My airbags did not deploy and my seat belts did not lock out. Directly after being hit my phone flashed a safety fracture malfunction on the dash board and then sent the notification to the mazda app on my phone.
When adjusting the driver seat, the vehicle began to display "Air Bag System Malfunction. Have the vehicle inspected." This was with roughly 20 total miles on the odometer. It is a new car - Mazda3 Turbo Sedan 2022. After researching online, this is a very common problem with new Mazda3s and there are many customers with the exact same issue. The customers have also made complaints to the NHTSA. Mazda has a technical service bulletin for this problem (Bulletin no:08-001/22) and states the issue is due to "Driver-side air bag module circuit: open circuit or resistance high". This issue also occurs on the passenger-side air bag. It is a possibility that I could have been in an accident and the side airbag would not function. Or even perhaps the air bag may have had an electrical error and engaged itself while I could have been driving. …
The vehicle was badly rear ended and the airbags didn't deploy. The dashboard indicated an "airbag malfunction" after the collision happened. The failure of the airbags put myself and my passengers at serious risk; if we weren't wearing our seatbelts, which we were, we would have been gravely injured. The vehicle is less than a year old and still under warranty.
My car started to show a warning on the dashboard that the airbag system is malfunctioning. The alarm would come on for a few seconds, then turn off, then repeat like that multiple times every drive. At the time it started, my car had less than 500 miles on it, and it continued after that until I got it fixed. I took it to the Mazda dealership where I bought it, and they said it was a short circuit to the driver's side airbags. They told me that could have caused the airbags to deploy prematurely or not at all in an accident. I am also a part of a Mazda 3 community and am active on the forums, and in the last month I have seen at least 30-40 other 2021 Mazda 3 owners also report the same exact error code, with the same short circuit issue being cited as the problem by the dealership. …
A few weeks ago the following error message popped up on my dashboard: "Airbag Malfunction. Have vehicle inspected". This message came and went intermittently whenever I had a passenger in the vehicle. I scheduled and appointment at my local Mazda dealership service center and in the meantime looked up the issue and found that it was extremely common and caused by a design flaw where the wire harness to the seat airbags was too short causing eventual failure. The issue was so prominent that Mazda even issued a service bulletin on how to fix it which included replacing the wire harness and adding extra security clips to help prevent future failure. So I get my car to the service center and after looking at it they confirm this exact issue, say it is not covered under any extended warranty and tell me that it will cost approximately $1,000 to fix (diagnostic, parts plus labor). …
1. What component or system failed or malfunctioned, and is it available for inspection upon request? The airbag system, specifically the curtain airbags and related airbag control components, malfunctioned by deploying spontaneously without any collision or impact. The vehicle and deployed airbags are still available and can be inspected upon request. 2. How was your safety or the safety of others put at risk? The spontaneous deployment of all four curtain airbags created an immediate safety hazard. Airbag deployment can cause injury, disorientation, or loss of control, especially if it occurs unexpectedly. Although the vehicle was stationary at the time, deployment without warning posed a serious risk of injury to occupants and could have resulted in a crash if it had occurred while driving. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? …
The contact owns a 2016 Mazda Mazda3. The contact stated while driving 40 MPH, the front driver's side door air bag and the front driver's seat air bag deployed. The contact sustained some busies due to the failure. The vehicle was taken to the dealer for inspection and diagnostic testing. The dealer inspected the vehicle and determined the failure was the result of an accident. The contact stated that the vehicle was inspected by Progressive Insurance and noted the failure as a mechanical failure by the manufacturer. The vehicle was not repaired. The manufacturer was made aware of the failure but did not assist. The failure mileage was approximately 126,000.
Air bags and bad tires - I hydroplaned on an interstate and totaled my car and another vehicle. I lost control and hit the other vehicle with the front of my car and continued to go up a 30ft embankment. On the way back down I took out a very large metal light pole with the front of my car and the air bags never deployed. After inspection of my car my tires were bad. I just bought the car brand new in 7 months ago.

NHTSA investigations
Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited.
