I was driving a 2024 Honda Prologue Elite and was coming to an ordinary stop at a safe distance behind another car at a red light. With my foot continuously and firmly pressed on the brake pedal, a brake alert/warning appeared on the dashboard screen accompanied by beeping, and the emergency brakes automatically engaged, stopping the car abruptly. Then, almost immediately, the vehicle suddenly lurched forward without warning, striking the vehicle ahead of me. I kept my foot firmly on the brake as the warning, beeping, and auto-brake activated again, and the vehicle lurched forward a second time, again striking the vehicle in front of me. This sequence repeated a third time with the same warning/beeping/braking and lurching pattern, causing a third forward impact. …
Shortly after purchase, I observed damaged/torn underhood insulation and reported the condition to the selling dealership, with concern that the vehicle should be inspected for related issues. Prior to a crash incident, I experienced concerns involving braking-related behavior and advanced driver assistance features, including lane assist and object/collision warning functions. During the crash, while reacting to a roadway hazard, the vehicle swerved and struck a pole, and I did not experience warning or intervention from driver assistance features as expected. Following the incident, a diagnostic pre-scan identified multiple faults involving brake control, radar sensors, image processing, and communication systems. Since the incident, similar concerns have continued, including recurring braking/driver assistance concerns, and the issues remain unresolved. …

On [redacted] Me and my son were in [redacted] in one of the basement garages. Parked successfully the first time inching the vehicle forward without issues and stopping as needed. 2nd run, got into parking spot got close and stopped 1 foot away from wall. Attempted to inch forward, lightly and quickly tapping on accelerator to disengage auto hold/brake and move forward but car did not respond. Repeated the action, quickly and lightly tapped again no response, then all of a sudden car lurched forward, I jammed on the brakes with no response and car hit the wall. Direct cause - Unknown. What I do know however was that the automatic emergency break system on my vehicle was activated, causing the vehicle to stop in the middle of driving. This resulted in the vehicle behind me hitting my car. Prior to the incident occurring, no warning indicators (lamps, messages, etc.) came from the dashboard. The vehicle just stopped. Immediately after this accident occurred. I turned off the forward collision safety feature, as I was scared of the issue (vehicle stopping) repeating itself. To date, my vehicle has not been repaired or my insurance agency contacted, as I do not believe I should be held responsible for the cost of this repair.
