Three (3) separate systems failed. 1. Upon seeing cars ahead of us slowing down, my husband pressed the breaks to slow down to avoid collision. When he pressed on the breaks, instead of slowing down or stopping the vehicle, the brakes failed to engage, and caused a surge that compelled the vehicle to lurch forward. The lurch propelled our vehicle into collision with another vehicle. 2. Also, the forward collision warning system failed to alert or engage the breaks. Our vehicle was stopped by the impact of hitting the other vehicle, not by its own breaks. 3. The automatic breaking system also failed to engage at all prior to impact.

Additionally, the contact stated that upon starting the vehicle, the messages "Parking Brake Malfunction", "Parking Brake Unavailable" and "Door Ajar" were displayed. The gear shift indicator light and the parking brake warning light were illuminated. The vehicle was taken back to the dealer, where it was diagnosed that the main wiring harness underneath the gear shifter was corroded. The dealer informed the contact the wiring harness would be replaced at the contact's expense. The contact stated that the failure occurred because there was no protective shielding around the wiring harness. …
multiple times this has been an issue. I stepped on the break and the car essentially leaps forward typically when going up down or straight. In November 2024 it happened while going up a hill slowly and I bumped into the vehicle ahead of me. My vehicle had no damage, but the car in front of me had to get a new skid plate and my insurance doubled because of it. Since the incident in November 2024, my car has unintentionally accelerated intermittently. finally, April 2025 this happened my car started shaking jolted forward, engine revving, and then my check engine light came on then immediately contacted my local dealership and scheduled an appointment to get my car in. They have had my vehicle since then and have said it is not safe to give it back to me so I am currently driving a loaner vehicle.
The contact owns a 2022 Toyota Sienna Hybrid. The contact stated that while driving 25 MPH, the vehicle crashed into the rear of a stationary vehicle; however, the air bags failed to deploy. Additionally, the ABS failed to activate as designed. The contact sustained a back injury upon impact; however, medical attention was not sought. The vehicle was towed to the residence. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 84,751.
The car will occassionally lurch forward while braking. It's somewhat random but most often happens when going down a slight hill. My best guess is that there is an issue with the transition from regenerative braking to mechanical braking. It feels like the vehicle momentarily looses contact with the brakes and that the brake pedal has to be pushed down another inch or two to reengage. My concern is that if this happens at the wrong time that I will get in an accident. This has happened ever since I bought it. It's not very often, maybe once a week. Speeds have been variable from under 25 to highway speeds.
The contact owns a 2021 Toyota Sienna Hybrid. The contact stated that while depressing the brake pedal, there was a three-second delay before the brakes would respond. Additionally, the brake pedal was difficult to depress, creating an extended stopping distance. The vehicle was taken to the local dealer who adjusted the brake stroke sensor and recalibrated the braking system; however, the failure persisted. The vehicle was later taken to a second dealer who bled the brake lines, but the failure persisted. The manufacturer was notified of the failure, but no assistance was offered. The failure mileage was 30,800. The VIN was not available.
FAILURE: Brake control system. Vehicle intermittently surges forward while braking at low speed, most often through a curve - the brakes momentarily release and the vehicle lurches before braking resumes. Appears to occur during the transition between regenerative and friction braking. Vehicle available for inspection. SAFETY RISK: Unpredictable momentary loss of braking at low speed, in parking lots, residential streets and curves where pedestrians and other vehicles are close. Driver cannot anticipate it. WARNING LAMPS: None. No lamp, no message, no stored trouble code. Present since early in ownership; purchased new September 2021. CONFIRMED BY DEALER: No. Reported to a Toyota dealer in 2022 under warranty. Dealer test drove, could not reproduce the intermittent condition, took no action. Also reported in writing to Toyota in August 2022; no substantive response. …
