On November 4, 2025, I believe my New Tesla (only 2 month old) had just updated to a new software version. That morning, when I tried to start the car to drive to Walgreens Pharmacy, the vehicle did not start - the screen was locked. I pressed the brake pedal several times, but there was no response. Then I pressed the accelerator and the brake again, and the car finally started. I believe i turned on the Full Self-Driving (FSD) mode as usual. The car backed out of my garage and moved only about ten houses down our street when I suddenly felt it was out of control, and within seconds, it hit a stationary vehicle on our street. I did not hear Forward Collision Warning or feel the Automatic Braking system engage. The tesla report told me that the collision warning failure and the Safety systems are affected, and the passage side (front right) tire shows 0 psi) . …
INCIDENT INVOLVING FSD (SUPERVISED) v14.2.2.4. On February 15, 2026, while the vehicle was operating under Full Self-Driving (Supervised) v14.2.2.4, the system failed to detect a stationary curb. The vehicle initiated a trajectory that resulted in a collision, causing a structural gouge in the passenger-side rear tire sidewall. Per MA 540 CMR 4.04, this damage renders the vehicle unroadworthy. I have saved Dashcam/Sentry video footage as proof of the system's failure to navigate the obstacle.
On [redacted], while operating my 2024 Tesla Model Y in Full Self-Driving (Supervised) mode, the vehicle experienced an Uncommanded Steering Input leading to a collision. The vehicle was traveling at a very low speed of approximately 5 mph in a clear, simple environment. Despite this walking pace, the FSD software failed to identify a visible concrete curb and initiated a sudden, sharp turn directly into the obstacle without any prior warning or driver input. The fact that the system executed such a violent and erroneous maneuver at 5 mph indicates a fundamental failure in the Neural Network's object detection and path planning logic. If this logic failure had occurred at higher speeds or near pedestrians, the results could have been catastrophic. The local Tesla Service Center (Watertown, MA) refused to conduct a physical inspection or a log review of the steering torque. …
My Tesla (dark gray) was t-boned by a 2015 Hyundai (white) while it was passing the intersection on green light. The Hyundai hit my car when it tried to turn right on red light. My car was running at around 25 mph at that moment. About one second before the accident, my car's front collision warning went off, but everything was too late; the Hyundai's left front bumper hit my car. The crash did not cause any serious damage on the white Hyundai; as you can see in the photo, it barely got any dents, except its left front headlight cap was broken. The incident created small dents on my car's right fender panel and front right door. However, even though my car's front right wheel was not directly hit, it fell off during the accident. If my car was running at a high speed, it would have rolled over and caused serious injuries.
The car suddenly crossed the double yellow line on a local road and hit another car coming from the opposite direction on a local road (Adams Lane, North Brunswick, NJ). The wheel joint was faulty that broke (see attached pictures) and pull the car to the other side of the road without warning. Even the airbag in this car is not deployed. The vehicle was towed by police to a garage (Coppa's Service Center & Towing, 1 Moore Road, North Brunswick, NJ 08902. ph. [phone]). The vehicle has been declared as a total loss by the insurance company. Because my car crossed the yellow line and hit the other car, I got tickets (crashdocs.org; Police Report # [redacted]; Date: 06/20/2023). I and the lone person the other care are lucky to survive this accident. I feel that such a fault might put other Tesla Model Y drivers/passengers and others in danger on the road. …
While making a left turn at normal turning speed, the steering system became unresponsive. The steering wheel was able to move; however, there was no corresponding movement of the front tires/wheels. The front wheels appeared to remain stuck at approximately a 45-degree left position and did not return toward the straight-ahead position despite steering input. It is also possible that the steering wheel itself became locked or partially locked during this event. As a result, the vehicle could not be corrected and collided with the median and a pillar. The collision caused the airbags to deploy and resulted in severe front-end damage, with an estimated repair cost exceeding $40,000. This steering failure occurred without warning and resulted in a sudden loss of vehicle control, creating a serious safety hazard to the occupants and surrounding traffic. …
I was sideswiped by another vehicle which just joined our direction of traffic coming out of a cutout. As soon as I saw the vehicle moving fast from inside the cutout to join our direction and into my lane, I immediately decided to move to the lane next to me. I was at the leftmost lane at the time. Halfway into the lane I just felt the violent impact of the other vehicle hitting us. My wife and I were shaken so bad, we were shocked and terrified at what just happened and what else might happen to us. I felt like we were travelling on the front wheels with our rear end up in the air. Next thing we knew, the car stopped in front of a tree. Our iphones and the our car's phone called 911. We were pulled out of the car and taken immediately into an ambulance and Orange Global Hospital, where we stayed for about 2 days. X rays, CT Scan and MRI were taken. …
