Vehicle roll over driver after being placed in park at the residence causing fatal injuries. Driver reversed out of garage and exited vehicle. Inspection at the scene by the sheriff's office revealed the vehicle was in park backed against some trees. This vehicle is equipped with a automatic transmission and a automatic emergency brake. The gear shift lever can be placed between park and reverse and not move. If the gear shift lever is moved slowly from drive to park or reverse to park the automatic emergency brake does not engage this condition is very dangerous.

The vehicle was taken to an unknown local dealer, but the cause of the failure could not be determined. The vehicle was taken to another dealer (bell Lexus north scottsdale, 18555 n. Scottsdale, scottsdale, az 85255) where it was diagnosed that the computer software needed to be updated. The contact took the vehicle back to the unknown local dealer and was informed that the failure could not be repaired. The vehicle was being serviced as a lemon law vehicle. The manufacturer was notified and confirmed the information provided by the dealer. The failure could not be remedied. The failure mileage was approximately 4,500.
This car experienced an unintended acceleration at the top of my driveway while being put into park. The car took off and hit the center posts of the two car garage and only stopped because it was caught on the supports in the foundation. We had experienced this sudden unintended acceleration several times during the first year we own the car when we were getting onto the highway. We did report it when we brought the car in for a service, they were aware of the complaint but at that time there was no remedy for the computer slash transmission issue. First problem with electrical was when the navigation screen went completely blank while driving during broad daylight. Don't know the date of this incident. Thought it was a reboot by enform. Thought nothing about it. On 12/21/2017, check electric brake warning light came on when I started my car in the morning after it had been sitting all night. Took into the dealer. Dealer could not find any problem with it or any error codes indicating any problem. They reset the error light. On 4/23/2018, I stopped for a stop sign at about 12:30 pm, nothing happened when I tried to accelerate at which point I noticed I had no lights on anywhere on my car. It was dead. My car caught fire on its own. I was awaken by the police who said that my parked car is on fire. The fire department and police confirmed that the fire started on its own from the engine and there is no larceny. They mentioned that there was a vehicle recall for the same problem and then I search online and found the recall. On feb 9 2016 a rodent had chew the wires inside my engine which the battery/check engine and the all wheel traction light came on. So I drove it to the dealer ship in tacoma fife of wa. So my insurance company told them to replace the whole engine main wiring harness for a total cost of $5372.38. Everything was good to go, then on 9 April 2016 my check engine and AWD drive traction light came on. So I took it to the dealer again, but while driving on the highway I heard chimes going off then all of a sudden it loss acceleration power and loss speed quickly. I was able to manuever off safely to the side out of traffic and down an exit to a complete stop. I turn off the vehicle and started it back on. The door lock accuator on the driver's side is inoperable.I have to manually lock the driver side door then open the passenger door and lean into the car and reach over to unlock the driver side door each time I need to open the door. Cost to repair is $600.00 (parts & labor) I feel this is excessive as the vehicle is only 5 years old and has only 55,500 miles! Parts dealer noted he keeps the part stocked on his shelf as it is frequently requested.
My 2013 Lexus AWD rx 350 has a faulty wire harness which has shorted out its GPS navigation system, radio, and media center. I replaced the headlamp on the passengers side and the blast, but the 15 amp fuse continues to blow out. Therefore, the faulty wire harness has caused my headlights, GPS navigation system, and radio to no longer work.
