On [redacted], while using Full Self-Driving (FSD) to drive to a hospital in Oak Park, Illinois, the vehicle experienced sudden unintended acceleration during the disengagement of FSD. As the vehicle approached the hospital area, I initiated disengagement of FSD to manually enter the parking area. During-or immediately after-the disengagement, the vehicle suddenly accelerated at full power without warning, causing an immediate loss of control and a collision with a nearby fixed structure. No system warnings or alerts were issued prior to the event, and the acceleration was so abrupt that manual intervention was not possible. This occurred in a low-speed, street-level environment where no acceleration was intended or expected and does not correspond to any normal driver input. …
The contact's wife owned a 2022 Tesla Y. The contact stated that while the vehicle was parked, his wife shifted the gear shifter into drive(D), and the vehicle independently accelerated. The Autonomous feature was not engaged. The brake pedal was depressed; however, the vehicle failed to respond as needed, and the front center of the vehicle crashed into a pile of cinder blocks, drove through a gate, and crashed into a dumpster, where it came to a stop. As a result of the impact, the front driver's side tire detached, and the dumpster was damaged. The contact stated that another person helped remove the driver from the vehicle due to the front driver door being unable to open. The air bags deployed as intended. No warning lights were illuminated. The vehicle was towed to a tow yard and was deemed a total loss. A police report was filed. …
While operating a 2022 Tesla Model Y with Full Self-Driving (FSD Supervised) engaged, the vehicle failed to execute a right merge onto the CA-280 North freeway ramp in Redwood City, CA. Instead, FSD initiated an unexpected wrong-way maneuver down a downward off-ramp. As the vehicle headed toward a sharp left curve, it failed to decelerate sufficiently for the turn. The vehicle maintained speed on the downward incline and then accelerated slightly, traveling at around 30 mph immediately prior to impact. Facing an active hazard initiated by the software, I performed an emergency manual override. System steering resistance and abrupt load transfer mid-turn caused an immediate loss of control, driving the vehicle over a mound and into an off-road ditch. Airbags deployed upon impact, and occupants (including a 5-month-old infant) were transported to the emergency room for evaluation. …
The contact rented from Turo a 2022 Tesla Model Y. The contact stated that while his daughter was driving at approximately 65-70 MPH in rainy weather, the contact's daughter released her foot from the accelerator pedal to decelerate; however, she lost control of the vehicle. The vehicle crashed into a guardrail and went into a ditch. The vehicle then inadvertently reversed onto the highway. The contact's daughter slammed her foot on the brake pedal, and the vehicle stopped immediately. No warning light was illuminated. No air bags were deployed. A police report was filed. The contact's daughter suffered body aches, and medical attention was received. The vehicle was towed to an unknown location. The contact was unaware if the vehicle was deemed totaled by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was unknown. The VIN was unavailable.
The contact owns a 2022 Tesla Model Y. The contact stated that while his wife was parking the vehicle, the brake pedal was depressed but the vehicle failed to stop as needed. The contact stated that the vehicle collided with a wall and was stopped by a curb. The air bags did not deploy. The vehicle was diagnosed by the manufacturer and the contact was informed that the accelerator had been depressed inadvertently; however, the contact was adamant that it was brake pedal that was depressed at the time of the failure. The vehicle was towed to a collision center. The vehicle was not repaired. The failure mileage was 10,444.
When driving home and try to make a left turn, the steering doesn't work and car suddenly speed up even though try to brake, the whole car is out of control, and the car hit my neighbor, and it's extremely dangerous! Thank god that no one is hurt. Police came and did a report but didn't say anything about Tesla. After that, I checked the alert history, nothing special, the only alert is remind me that window cleaner is running low. I tried to contact Tesla but failed, no one answer the phone call, no where to report. Everything is too bad
NHTSA investigations
Sudden Unintended Acceleration
The Office of Defects Investigation (ODI) received a petition requesting that ODI reevaluate its decision to deny DP20-001 on the basis that intermittent high electrical current demands on the vehicles' 12VDC systems may have caused some or all of the incidents examined by ODI in DP20-001. The petitioner bases this information on a review of open-source research and the DP20-001 denial.
