What owners report

On April 1, 2026, while pulling into a parking space at our church, I was applying the brake when the vehicle suddenly and forcefully surged forward without warning. The unintended acceleration was intense and unexpected, occurring despite continuous braking input. This resulted in the vehicle striking the building and causing significant damage to both the vehicle and the structure. This incident has occurred twice within a 10 day period, both times during low-speed parking/stopping maneuvers. There were no warning lights, alerts, no automatic emergency braking engagement, no air bag deployment, or prior indications of malfunction. I did report this to the police and they filed an Incident Report. Lexus was notified and an independent analysis engineer examined the data from the vehicle's CDR System. This report was sent to me and to Lexus.

On November 11, 2025, while reversing inside a garage, the rearview camera image- which had initially displayed correctly on the vehicle's screen-suddenly froze while the vehicle remained in reverse mode. At the same time, the Pre-Collision System (PCS) failed to engage the brakes while the vehicle was traveling at approximately 2-3 miles per hour. As a result, the vehicle struck the garage structure and sustained damage. Toyota investigated this incident (Case #[redacted]) but based its conclusion on the incorrect assumption that I had reported either a brake system malfunction or a complete absence of an image on the display (a dark or blank screen). Because of this, Toyota determined that it could not provide assistance regarding the resulting vehicle damage. This characterization is inaccurate. The issue was not a blank screen. …

The car comes with proactive driver assist. The system when activated will cause the car to unexpectedly turn the wheel and lunge usually left if the system suspects you are going off of track. It works even on unmarked 2 lane country roads. Very disconcerting and dangerous. My wife was then subsequently in a serious accident on a slippery and snowy night that totaled the car. We are not convinced that the proactive driver assist system was the cause of the accident but can't help but think it may have been a contributing factor to the accident. Her car swerved into the oncoming lane on a unmarked (snow covered) highway.

The contact owns a 2024 Lexus RX350H. The contact stated that while the vehicle was started, the Forward Collision Avoidance system and air bag warning lights illuminated. The vehicle was taken to a dealer for 17 days, where it was diagnosed that a software update was needed. The vehicle was not repaired. The manufacturer was made aware of the failure and the contact was informed that there was no fix for the failure. The failure mileage was 1,020.

My RX350 experienced malfunction of the Advanced Driver Assistance Systems (ADAS). Messages regarding the malfunction of the ADAS first appeared around mid-August 2024. Upon inspection during the week of August 27 2024, the Lexus dealer, Akron-Canton of Lexus, indicated that the malfunction was caused by rats chewing on the soy-based wiring. The dealership replaced the wiring at a cost of $1,295.10. This is a problem caused by faulty design and inappropriate parts and design. It is inconceivable that car parts would be made of animal food. This put me and my family at serious risk of accident, as my reliance on warnings was severely compromised. Lexus needs to immediately replace the soy-covered wiring with something that is not edible by rodents or any other wildlife. …

2023 RX · Adaptive cruise control · Aug 2024

When driving Lexus 2023 RXH with all safety except lane assist features, it suddenly veers to the right under specific conditions below - Have enabled ProActive Driving Assist - The car is travelling on a straight road, and has line markings (yellow left side, white right side) - There is a right turn to the side of the road, and the white shoulder markings taper off and merge to the side of the road (where there is a right turn) The car suddenly veers right and follows the white shoulder marking ignoring the yellow central line and the straight road. Seems very dangerous. Note that Lane Keeping Assistance is disabled (but I checked it since it needs something checked), Adaptive Cruise Contro is not engaged (inner road), but other options are all enabled, including Lexus feature called ProActiveDriving Assist. …

On July 15, 2022, my 2022 RX450hL drove itself across approximately 150 feet of rugged desert terrain and ended upside down in my neighbor's driveway. No one was in the car and no one was in the driveway where it landed. I had parked the car on the side of my house, after several seconds the car took off across the desert to self-destruct. I contacted Lexus, someone from their legal dept. contacted me and they sent a representative to collect the "black box." After that Lexus refused to take my calls or provide any information to me about the "black box." I had taken possession of the car 34 days when this occurred, it was brand new. This car was equipped with safety features that should have stopped it from driving as it continued over sharp, protruding rocks, went through cactus plants taller than me and over a cliff. …

2022 RX · Automatic emergency braking · Jul 2022 · crash

HI, I purchased a brand new 2022 Lexus RX350 from Hendrick Lexus of Charleston SC. Since day one I had dangerous issues below: 1. The transmission shifts up and down so hard, I've almost gotten rear ended due to the car and not braking. This I was told by two separate Lexus dealers I'm feeling a hard "transmission gap"-whatever that means. To add fuel to this dangerous defect is my autistic son would need to be specially medicated to ride in the car b/c he panics and tries to jump out. 2. The car also is equipped with a "rear kick sensor" that never worked, The service manager in Charleston kicked my bumper and swore at the car. I had another Service tech on video for minutes unable to open it. 3. The car, scarily, will not start on me or recognize the key. I've been 3 hrs from home and nearly stuck in a grocery store parking lot with my autistic son freaking out again. …
