Owner reports
Selected complaints filed with NHTSA by 2008 Toyota Yaris owners, lightly cleaned up (personal details are redacted by NHTSA).
My fiancee bought her 2008 Toyota Yaris on about the 26 Apr 2021. Right off she had brake failure pretty quickly after buying the car. Brake pads were suspected of failing. When the front tires were removed the calipers had built up thick rust on inside surface where the pads sit on both front calipers. The pads were pushed into the rotors heavily causing the driver side rotor to be damaged beyond acceptable limits. The right side rotor was turned successfully with enough rotor minimum thickness remaining. So the rust caused me to replace 1 rotor, both calipers and pads on front and rear brakes. The rust built up on the calipers threaded input and output brake fluid lines threaded attach points. This could have cause a fluid leak that could of been very dangerous leading to injury or death of occupants. I have not reported this to The Toyota dealership or anyone else. …

I tried to stop + could not. If feel as if there are several recalls on this vehicle, one that does not meet highway standards, in which the dealership should have known about before selling to me on October 31, 2016. I had trouble getting my title from the dealership + had several issues with the vehicle + it was within the 30 day warranty period, + they told me every time that everything was OK, until I was in my accident + now have + am still dealing with several medical issues.

I was going 15-20 MPH on on ramp to the freeway and accidentally veered right hitting the guard rail on the right front side of the car. None of the airbags deployed and my seatbelt did not lock, resulting in my face hitting the steering wheel, breaking my nose and cutting my lip.
Airbags did not deploy in passenger-side car accident. Neither passenger airbags nor driver airbags.
While my daughters were travelling north on the freeway an oncoming vehicle driven by an elderly lady entered the freeway going in the wrong direction. It was night time and it was raining. The cars in front of them started responding in various ways trying to avoid this vehicle. There were a number of crashes. Our vehicle hit the side rail and the vehicle in front of us . The car hit had quite a lot of damage and our vehicle was totaled . It was deemed a no fault accident on our part due to the nature of the incident. Our Yaris was totaled. Completely smashed in front and sides, driver door would not even open yet no air bags deployed. None. The police officer, tow truck driver, other individuals also involved in the incident , as well as my daughters all witnesses to the state of the vehicle, were shocked with this. The vehicle was even difficult to tow. …

NHTSA investigations
Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited.
