NHTSA investigation EA21002 · opened Sep 17, 2021 · open

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited.

A caltrans (or san jose) freeway safety car came by and put about 2 gallons of gas in my tank, but as soon as the engine started we both smelled raw gasoline. I then drove about 1/8 of a mile to the nearest parking lot and stopped, and we both smelled very strong gasoline odor and found fuel leaking onto the ground, where it pooled in a large puddle. There was no fire, but easily could have been. …
